Lumen Dental Studio LLC is the controller of personal data collected through this website. For treatment records we also follow our separate Notice of Privacy Practices, available at the front desk and on request, as required by HIPAA.

• Appointment details: visit type, preferred time and any notes you add.
• Insurance details, if you choose to share them before your visit.
• Technical data: browser type and pages visited, only if you allow analytics cookies.
3. How we use it
• To schedule, confirm and remind you of appointments.
• To check insurance benefits you ask us to verify.
• To answer your questions and send forms before your visit.
• To send our monthly newsletter, only if you subscribe.
We never sell your personal information.
4. Cookies
We use strictly necessary cookies to make booking work and remember your consent choice. Analytics and marketing cookies are used only if you accept them in the cookie banner. You can change your choice at any time via Cookie settings.

We share data only with service providers that help us run the practice (appointment software, text-message reminders, payment processing) under contracts that protect your information, and with insurers when you ask us to file a claim.

Booking and contact records are kept for up to 3 years after your last visit. Clinical records are kept for the period required by state law.

• Access a copy of the information we hold about you.
• Ask us to correct or delete it, where the law allows.
• Withdraw consent to marketing at any time.
• Complain to the relevant data protection or health privacy authority.
8. Security
Our site uses encrypted connections (HTTPS), and patient systems use access controls and audit logs. No online system is completely secure, so please avoid sending detailed medical information by email or web form.
